In the 1950s in the Midwest of the USA: The young girl Baby Doll (Emily Browning) is blocked by their wicked step-father in a mental hospital. There she should get a five-day lobotomy, which leads her to a personality change. To better endure the pain, flees Baby Doll in an alternate fantasy world and plans her escape in this world.
